Job summary

Blue Stone Therapy in Austin, MN is seeking a licensed Occupational Therapist for a full-time, permanent position in a Skilled Nursing Facility/Long-Term Care setting. Compensation is $36.00-$55.00 per hour with a sign-on bonus up to $10,000 for new full-time hires.

Qualifications

• Must be a licensed Occupational Therapist to apply.
• Current license as an Occupational Therapist in the state of practice.
• NBCOT certification may be required depending on state rules.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Ensure all physician orders are obtained and up to date.
