Job summary

Centra Healthcare Solutions Inc. is seeking an Occupational Therapist (OT) for a permanent, full-time role in a Skilled Nursing Facility in California. The OT will assess referred patients, develop and administer rehabilitation programs under physician orders, and set realistic goals based on each patient’s abilities.

Documentation and family education are required components of care. Requirements include graduation from an accredited OT program, active state licensure (or in process), and CPR

Qualifications

• Graduated from an accredited occupational therapy program.
• Active state OT license or in process for licensure.
• Current CPR certification.
• Provide skilled OT services/interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care for OT interventions.

Responsibilities

• Assess referred patients to determine rehabilitation needs and goals.
• Establish and administer treatment programs with physician-directed goals.
• Set realistic and achievable goals based on patient capacity and tolerance.
• Document patient condition and progress; educate patients and families.
