Qualifications

• Active and valid Texas Occupational Therapy Assistant license.
• COTA credential required.
• In good standing with all regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Knowledge of Net Health electronic documentation is a plus.
• Strong communication and teamwork skills are essential.

Responsibilities

• Plan and administer occupational therapy treatments to residents to relieve pain, restore or improve function.
• Provide skilled OT services under the supervision of an Occupational Therapist per physician orders.
• Respond to service requests by relaying information and referrals to the OT.
• Ensure all treatments align with the established plan of care.
• Provide clinical support and instruction to patients, families, and caregivers.
• Monitor patient response and report changes as appropriate.
• Complete required documentation promptly per policy and regulations.
• Attend required Rehab Director meetings.
