Job summary

The Lakes Home Care Services is seeking an Occupational Therapist Assistant to join our team, delivering care in patients' homes and outpatient settings, helping individuals improve daily living and functional independence.

As part of a dedicated home health team, you will assess patients, design personalized plans, guide families, and coordinate with healthcare professionals to ensure holistic, compassionate care.

Qualifications

• Outpatient experience required.
• Strong knowledge of anatomy, physiology and medical terminology.
• Experience with ADLs and patient care in clinic/home health.

Responsibilities

• Conduct patient assessments to determine needs and goals.
• Develop personalized treatment plans to improve patients' abilities to perform daily activities.
• Provide therapeutic exercises and activities to enhance patients' motor skills and coordination.
• Educate patients and families on adaptive techniques and equipment.
• Collaborate with healthcare professionals to ensure comprehensive patient care.
