Job summary

The Lakes Home Care, Inc. is seeking an Occupational Therapist Assistant to join our team providing in-home outpatient care.

You will help individuals improve daily living and functional independence across various home settings. The OTA will assess patients, create personalized treatment plans, and guide families on ADLs and adaptive equipment, collaborating with healthcare professionals to ensure comprehensive patient care.

Qualifications

• Must have outpatient/home health experience and ability to assess patients and develop care plans.
• Strong knowledge of anatomy and physiology relevant to occupational therapy.
• Familiarity with medical terminology and ADLs in home health settings.
• Experience collaborating with other healthcare professionals to coordinate patient care.

Responsibilities

• Conduct patient assessments to determine needs and goals.
