Job summary

Carle Health is seeking an RN Navigator to coordinate surgical patient care across specialties. The role combines clinical expertise with advocacy to help patients navigate the healthcare system and ensure optimal outcomes.

The RN Navigator collaborates with interdisciplinary teams, supports program development, quality improvement, and education initiatives, and ensures seamless care across treatment continuum within Carle Health’s network.

Qualifications

• RN license (state as applicable)
• BLS certification within 30 days
• National Nursing Certification
• BSN degree
• 2 years orthopedic RN experience
• Strong communication, organizational and critical-thinking skills
• Proficiency with EMR and data management systems
• Ability to work autonomously while collaborating within a team

Responsibilities

• Coordinate care across physicians, nursing staff, and patients to improve continuity of care.
• Guide patients through the care process from diagnosis to follow-up for positive outcomes.
• Arrange referrals, scheduling and follow-ups to support longitudinal care.
• Apply the nursing process to provide direct patient care as needed.
• Develop and update individualized, patient-centered care plans.
• Educate patients and families about diagnoses, treatments, medications, and resources.
• Create educational materials tailored to patient needs.
• Collaborate with leadership to develop clinical practice guidelines and care protocols.
• Monitor quality metrics and patient outcomes to inform improvements.
• Participate in community outreach and multidisciplinary clinics.
• Maintain clear communication with patients, families, and healthcare teams.
• Mentor staff and support ongoing education and competency development.
