Job summary

Carle Health is seeking an RN Navigator to coordinate comprehensive surgical patient care across specialties, combining clinical expertise with advocacy to guide patients through diagnosis, treatment, and follow-up.

The RN will collaborate with physicians and interdisciplinary teams, develop care plans, educate patients and families, monitor quality metrics, and participate in program development and outreach, while adhering to policies and standards.

Qualifications

• Basic Life Support (BLS) within 30 days - American Heart Association (AHA).
• Licensed Registered Nurse (RN).
• BSN required. Two years' orthopedic RN experience.
• Strong communication, organizational, and critical-thinking skills.
• Proficiency with electronic medical records (EMR) and data systems.
• Ability to work autonomously and in a team.

Responsibilities

• Coordinate care across physicians, nursing, and staff to ensure seamless care.
• Guide patients through diagnosis, surgical treatment, and follow-up.
• Facilitate referrals, scheduling, and follow-up appointments.
