Job summary

Yale NewHaven Health is seeking a Registered Nurse to assess and coordinate patient care on an orthopedic unit. You will supervise non‑RN personnel, participate in charge nurse duties, and ensure high-quality patient outcomes.

The role requires CT RN licensure, BLS certification, and proficiency with electronic patient records. You will engage in professional development and collaborate with a multidisciplinary team to maintain excellent care standards.

Qualifications

• Valid State Of Connecticut Registered Nurse License
• Proficient In Use Of Computer Software To Include Electronic Patient Records
• BLS Certification Required

Responsibilities

• Assess the patient’s physical and psychosocial condition on an ongoing basis throughout the continuum of care.
• Develop, communicate and document a plan of care reflecting patient problems and interventions with patient, family and healthcare team.
• Implement and coordinate the plan of care in collaboration with the healthcare team.
• Evaluate the effectiveness of the plan of care and readiness for discharge; revise as needed and make referrals.
• Educate patients and families after assessing learning needs and readiness; document outcomes.
• Function as a preceptor to new staff and support professional development.
