Responsibilities

• Develops productive relationships with patients, families and care teams.
• Demonstrates cultural awareness and accountability within the team.
• Provides teaching to patients and families to guide care and transition home.
• Supports development of students and new staff; may serve as a preceptor.
• Formulates daily goals and a plan of care with patient partnership.
• Communicates with care providers and documents patient information clearly.
• Creates a caring patient-focused experience and builds healing relationships.
• Participates in department quality improvement and unit projects.
• Takes personal responsibility to improve patient satisfaction with care and service.
