Job summary

Qualifications
- Current RN license in state of practice.
- BSN within four years of hire or promotion; degree verified.
- Demonstrated disease-specific care expertise.
- Experience with quality management programs and registry data.
- Proficient in graphs, tables, and trend reporting.
- Strong critical thinking, data organization and analysis.
- Experience developing educational programs.
Responsibilities
- Coordinate care across the continuum and develop clinical protocols.
- Plan and implement practice management guidelines and monitor care.
- Provide managerial oversight for program personnel and budgets.
- Act as a consultant/liaison for stakeholders.
- Evaluate program effectiveness and drive quality improvement.
