Job summary

Wellstar Health System in Georgia seeks a licensed Physical Therapist to assess, plan and implement rehabilitative programs that improve mobility, relieve pain and prevent deformity. You will work across the care continuum in a cost-conscious, quality-focused setting.

Responsibilities include treatment planning, patient assessment, documentation, discharge planning and patient education, with collaboration across the care team to ensure safe, effective care and ongoing professional development.

Qualifications

• DPT/MSPT/BPT degree required.
• Active PT license required upon hire.
• BLS certification required.

Responsibilities

• Implements the physical therapy treatment plan.
• Selects appropriate treatment activities to progress patient towards goals.
• Revises goals/plan of care with patient/family input.
• Provides discharge instructions and referrals as appropriate.
• Acts as a referral source for staff and physicians on rehab services.
• Documents evaluation and plan of care per guidelines.
• Maintains timely patient education and documentation.
