Job summary

Penn Medicine’s LG Health system in Lancaster, PA is seeking a seasoned registered nurse to provide clinical navigation and coordinate inpatient and outpatient care. You will deliver patient education, develop care plans, and collaborate with physicians and health-care teams.

Requirements include an active RN license in Pennsylvania, BSN, 3+ years in a related specialty, CPR certification within 30 days of hire, and eligibility to participate in Medicare/Medicaid programs.

Qualifications

• Active Registered Nurse license in PA
• BSN required
• 3+ years of clinical nursing experience in related specialty
• CPR certification (AHA BLS) within 30 days of hire
• Good standing with Medicare/Medicaid and federal/state programs

Responsibilities

• Provide clinical navigation for patients within the Penn Medicine LG Health system
• Coordinate inpatient and/or outpatient care needs and develop care plans
• Educate patients and caregivers on disease management, treatment options and self-care
• Collaborate with physicians and health-care teams to ensure continuity of care and references
