Formación

• Bachelor's degree in nursing (BSN) is required.
• Current Illinois Registered Nurse license is required.
• BLS certification through American Heart Association is required.

Responsabilidades

• Performs comprehensive patient assessments with documented outcomes.
• Communicates changes in patient condition promptly to the care team.
• Collaborates with patient, family and health professionals to plan care and discharge needs.
• Performs discharge planning and appropriate referrals.
• Maintains medication reconciliation and patient education at bedside.
