CereCore is seeking a Pharmacist who will work Remotely for our client based in Indiana.

Summary

The COE Pharmacist is responsible for monitoring, evaluating, and making recommendations to ensure the appropriate and effective use of pharmaceuticals for patient care. The COE pharmacist shall use the medication process to assess, plan, intervene when necessary, implement, and evaluate patient care in compliance with client hospital policies, protocols, and procedures.

Requirements
  • Approving orders, processing orders, validating orders, and clinical dosing, managing high alerts vigilance at night.
  • Provides patient care activities to ensure safe and effective drug therapy.
  • Serves patients, physicians, nurses, and other pharmacists.
  • Maintains and improves job knowledge, skills, and competence.
  • Practices and adheres to the “Code of Conduct” philosophy and “Mission and Values Statement”.
  • Performs duties according to standards of care, evidenced by completing a competency skills checklist.
  • Supports continuous quality improvement of patient care.
  • Prepares reports at shift changes to communicate incomplete tasks and safety concerns.
  • Demonstrates mastery of required functions, communication, and systems.
  • Enters orders accurately and timely into the computer.
  • Supports departmental goals for order entry and clinical programs.
  • Follows procedures for non-formulary, target, restricted, and investigational drugs.
  • Adjusts assignments based on workload and resources.
  • Screen for drug interactions, allergies, duplications, and appropriate diagnosis prior to order entry.
  • Addresses facility queues promptly.
  • Reports adverse drug events and medication incidents.
  • Disseminates drug information timely for quality and cost-effective outcomes.
  • Reviews and interprets physician orders using patient profiles.
  • Monitors IV drug compatibility, concentration, and rate.
  • Participates in quality improvement programs.
  • Assists in developing and reviewing COE protocols.
  • Documents interventions and follow-up actions.
  • Assesses orders for age-specific appropriateness from neonatal to geriatric.
  • Follows pharmacy policies and procedures.
Minimum Requirements
  • Indiana Pharmacist License.
  • Bachelor’s degree in pharmacy; Pharm D preferred.
  • One year hospital experience preferred.
  • Experience with MEDITECH and Vigilance preferred.
  • Ability to adapt to changes and build strategic relationships.
  • Strong teamwork and customer focus.
  • Up-to-date professional knowledge and high work standards.
