Job summary

Jefferson Health Northeast is seeking a dedicated Registered Nurse to deliver patient-centered care, develop individualized care plans, and collaborate with a multidisciplinary team in a high-demand setting.

The role requires active RN license and BLS, with strong focus on education for patients and families, discharge planning, and accurate health information documentation to ensure safe transitions of care.

Qualifications

• Specialized Diploma required from an accredited nursing program.
• Knowledge of patient- and family-centered practice and collaboration.
• Cultural awareness and ability to respect diverse backgrounds and values.
• Ability to communicate effectively and collaborate with patients, families, and teams.
• Knowledge of patient assessment, care planning, and discharge planning.
• Knowledge of medication administration and prescription processes.
• Understanding of health information documentation and accurate record-keeping.
• BLS certification (AHA) is required; RN license in state of practice.

Responsibilities

• Interacts with co-workers, patients, their family/designated care partner(s), and other staff to deliver quality healthcare.
• Partners with patients and families in identifying learning needs and provides education.
• Uses information technology to communicate, manage knowledge, monitor outcomes, and support ethical decision making.
• Works within nursing and inter-professional teams with open communication and shared decision-making.
• Contributes to safety, quality improvement committees, and ongoing professional development.
