Qualifications

Responsibilities

• Assess patients’ physical, social, and psychological status and gather information from the care team.
• Plan individualized care with patient, family, and team input and coordinate education and follow-up.
• Implement care and coordinate the interdisciplinary team to execute the plan.
• Evaluate patient responses and revise the care plan as needed; document outcomes.
• Uphold professionalism, regulatory standards, licensure scope, and accurate documentation.
