Job summary

Qualifications

Responsibilities
- Assess patient conditions and create individualized care plans.
- Collaborate with interdisciplinary team for continuous care.
- Administer medications and treatments per protocols.
- Monitor patient progress and adjust plans as needed.
- Document care activities accurately in medical records.
- Educate patients and families about conditions and post-discharge care.
- Advocate for patient safety and prevent infections.
- Coordinate with healthcare team for effective care delivery.
- Perform therapeutic and diagnostic interventions as required.
- Provide emotional support to patients and families.
- Engage in quality improvement initiatives.
