Qualifications

• Specialized Diploma Graduate of an accredited school of nursing.
• Knowledge of patient- and family-centered care and nursing processes.
• Active RN license in state of practice.

Responsibilities

• Interacts with co-workers, patients, their family/designated care partner(s), and other staff per Jefferson Health values.
• Education for patients and families, identifying learning needs and priorities.
• Uses information technology to communicate, manage knowledge, and monitor outcomes of care.
• Functions effectively within nursing and inter-professional teams and supports shared decision-making.
• Participates in safety and quality improvement committees and integrates feedback into practice.
• Assists in discharge planning and continuation of care after hospital stay.
