Responsibilities

• Collects data in a systematic and ongoing process; issues are identified and diagnoses are developed based on assessment data.
• Develops an individualized plan with strategies that address diagnoses and coordinates care with the team.
• Implements the plan safely and timely; documents changes or omissions and refers to appropriate providers.
• Consults with healthcare providers and makes referrals as appropriate; uses community resources to implement the plan.
• Documents required information in a timely fashion with correct terminology and formatting.
• Engages in quality improvement activities to improve nursing care and patient outcomes.
