Job summary

Qualifications

Responsibilities

• Perform comprehensive patient assessments using a systematic approach.
• Identify patient needs and nursing diagnoses from assessment data.
• Develop, implement, and evaluate individualized care plans with outcomes.
• Prioritize care based on patient condition.
• Coordinate care across healthcare team and settings.
• Implement nursing interventions safely and promptly.
• Document assessments, interventions, outcomes and changes to care plans.
• Collaborate with providers, patients, families, and teams.
• Engage patients and families in planning and decision-making.
• Monitor responses and modify plans as needed.
• Apply evidence-based practice and quality improvement in care.
