• Performs comprehensive patient assessment via interview, observation and physical exam.
• Develops and implements an individualized care plan with patient/family and health care team.
• Documents progress and outcomes; updates plan as condition changes.
• Coordinates interdisciplinary care and communicates with team to meet outcomes.
• Demonstrates safety, appropriate use of lift equipment, and patient handling.
• Delivers patient bedside lab testing and quality control as ordered by medical provider.
• Provides shift handoff reports and education to patient and family as needed.
• Maintains professional accountability and ongoing self-development.
