Community Mental Health Affiliates, Inc. in New Britain, CT is seeking an Open Access Intake Clinician II to join our Adult Outpatient program. You will assess clients, develop treatment plans, and coordinate with a multidisciplinary team to provide evidence-based care.

The role requires a master’s in behavioral health, CT licensure, and experience with EMR systems. Bilingual (Spanish/English) candidates are preferred; a CT driver’s license is required.

Qualifications

• Master's degree in behavioral health required.
• Connecticut license as LCSW/LMFT/LPC/Ph.D./LADC as appropriate for services.
• Bi-lingual preferred (Spanish/English).
• Valid Connecticut Driver’s License.

Responsabilités

• Maintains trainings and certifications as required by program model.
• Performs comprehensive intake assessment with diagnosis and treatment recommendations.
• Completes assessment tools and develops clinical formulation within timeframe.
• Performs individual, family and/or group treatment using evidence-based practices.
• Develops goals with client with measurable objectives and monitors progress.
• Collaborates with other providers and communicates clinical information timely.
• Responds to crisis situations and triages appropriately; coordinates with providers.
