Health Solutions is hiring an Open Access Clinician for Crestone Recovery Services in Pueblo, CO. The role involves conducting assessments, developing initial treatment plans, and linking clients with resources to reduce risk, while collaborating with probation, parole, and social services partners.

The position requires a master’s degree in a clinical field, Colorado licensure or candidate status within 2 years, and a commitment to high-quality, compassionate care.

Qualifications

• Master's degree in social work, counseling, psychology, or a closely related field.
• Colorado clinical license (LPC, LCSW, LMFT) or ability to obtain candidate status with DORA within 2 years of hire.
• Experience working successfully with a multi-disciplinary team.
• Computer proficiency.
• Valid Colorado driver’s license and transportation during work hours.

Responsibilities

• Conducts substance abuse/mental health/co-occurring assessments for adults, adolescents and children.
• Provides case management services.
• Maintains accurate records (written and electronic).
• Participates in collaborative treatment planning with clients.
• Provides exceptional clinical care to all clients.
• Timely documentation of clinical service.
• Manage caseload and discharge appropriately.
• Participates in team and case staff meetings; consults with team members as needed.
