Health Solutions is seeking a Clinician to join the Youth & Family Services team at 1012 W. Abriendo in Pueblo, CO.

This full-time role focuses on outpatient and home-based case management and therapy for youth and families, offering meaningful opportunities to improve community wellbeing. Responsibilities include intake, assessment, treatment planning, collaboration with community providers, and maintaining accurate records.

Qualifications

• Graduate degree in social work, psychology, or a closely related field.
• Demonstrated computer proficiency and experience with electronic health records.
• Colorado driver’s license and access to transportation during work hours.

Responsibilities

• Provides intake and assessment, including professional interpretation and clinical analysis to develop treatment plan.
• Provides case management services, collaborative and integrative services with resources in the community.
• Provides assessments and treatment planning for clients who are seeking residential treatment.
• Monitors client placement, discharge planning and identifying step-down services to youth and their family.
• Keeps accurate, complete and up-to-date records with 100% compliance with Center standards.
• Maintains competency and proficiency with the agency’s Electronic Health Record.
• Works in collaboration with community providers as well as health care professionals.
• Requires some evening and on-call hours.
• Performs other job-related duties as assigned by a Program Director or Supervisor
