Indiana University Health, Inc. is seeking a Social Worker (BSW/LBSW) at Arnett Hospital – AOB. This full-time, day-shift role focuses on psychosocial support for patients and families during hospitalization, guiding discharge planning and coordinating care across services.

40 hours/week, no weekends or holidays with occasional shifts. The role requires a BSW and at least 3 years of experience, with Indiana LBSW licensure required by Oct 5, 2025.

Formación

• BSW degree required.
• 3 years of experience required.
• Permanent LBSW licensure in Indiana required by 5 Oct 2025; incumbents before then must obtain within 6 months; LSW license holders must maintain licensure.

Responsabilidades

• Meet face-to-face with patients and families to assess psychosocial needs and provide support.
• Interview patients and relatives to gather social histories for care planning.
• Identify and help overcome barriers to care and discharge planning.
• Connect patients with social support programs and community resources.
• Coordinate discharge plans to ensure safe transitions of care.
• Serve as liaison between medical staff, patients, families, and outside agencies.
• Collaborate with multidisciplinary team to plan patient care needs.
• Adhere to state and county adult/child protective mandates; coordinate with protective services as needed.
• Adapt services for diverse populations.
• Document clinical activities in the electronic medical record.
