Indiana University Health is seeking a Social Worker – BSW, LBSW at Arnett Hospital (Onsite). Full-Time Day Shift, Mon–Fri, 40 hours/wk, 8:00 AM–4:30 PM with no weekends or holidays. Flexible shifts available.

This role supports patients and families facing psychosocial challenges, coordinating discharge planning and connecting to community resources. Requires a BSW, 3 years' experience, and Indiana LBSW licensure by Oct 5, 2025 if applicable.

Qualifications

• BSW degree is required.
• 3 years of experience required.
• Permanent LBSW licensure in Indiana required for all new hires as of October 5, 2025; incumbents must obtain in 6 months; LSW must maintain licensure.
• Strong assessment, resource development, and discharge planning skills.
• Strong communication, planning, and collaboration within a multidisciplinary team.
• Knowledge of state and county adult and child protective legal mandates.
• Knowledge of growth and development across the lifespan.
• Experience adapting services to diverse populations.
• Computer literacy and proficiency in EMR documentation.

Responsibilities

• Meet face-to-face with patients and families to assess psychosocial needs and provide support related to medical conditions and hospitalization.
• Interview patients and relatives to obtain social history relevant to medical problems and care planning.
• Identify and help patients overcome barriers to obtain maximum benefit from medical care.
• Connect patients with social support programs, community resources, and other appropriate resources.
• Develop and coordinate discharge plans to support safe transitions of care.
• Serve as a liaison between medical and nursing staff, patients, relatives, and outside agencies.
• Collaborate with the multidisciplinary team to plan and communicate patient care needs.
• Apply knowledge of state and county adult and child protective legal mandates, coordinating with protective services as needed.
• Adapt services to meet the needs of geographically, culturally, and financially diverse populations.
• Document all clinical activities accurately in the electronic medical record.
