Qualifications

• Graduated from an accredited school of pharmacy is required.
• A pharmacist license in the state of Oregon is required.
• Registered Pharmacist (RPh) is required.
• All licensure and certifications necessary to perform duties as a Pharmacist in Oregon are required.
• Doctor of Pharmacy (PharmD) degree is preferred.

Responsibilities

• Verify and fill medication orders.
• Monitor patient medication therapies.
• Provide drug information to patients and staff.
• Identify and troubleshoot medication safety concerns.
• Document interactions for medical review.
• Supervise and direct support staff.
