Job summary

Vantage Health Partners in Berea, Kentucky seeks a Primary Care Nurse Practitioner to join our clinical team on-site. This full-time role provides comprehensive, patient-centered primary care across the lifespan within a hospital/healthcare setting.

You will independently assess, diagnose, and manage patients, develop individualized treatment plans, manage chronic diseases, provide preventive care, and collaborate with physicians and nursing staff to ensure seamless, high-quality care.

Qualifications

• MSN or DNP degree required.
• Active Kentucky APRN license.
• FNP or AGPCNP board certification.

Responsibilities

• Conduct comprehensive histories, exams, and health assessments.
• Diagnose and treat acute and chronic illnesses per guidelines.
• Develop and monitor individualized care plans.
• Manage chronic diseases such as hypertension, diabetes, asthma.
• Prescribe medications and order tests as appropriate.
• Educate patients on prevention and self-management.
• Document encounters in EMR and coordinate care.
