Vantage Health Partners in Ashland, KY is seeking a Primary Care Nurse Practitioner to join our clinical team on-site. This full-time role requires five days a week and focuses on delivering comprehensive, patient-centered primary care across the lifespan.

You will assess, diagnose, and manage patients, develop treatment plans, and collaborate with an interdisciplinary team to improve community health outcomes. Strong emphasis on preventive care and patient education.

Formación

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP).
• Active APRN/NP license in Kentucky.
• National NP board certification (FNP or AGPCNP).
• Demonstrated expertise in primary care patient assessment, diagnosis, and treatment.
• Proven ability to manage chronic disease effectively.
• Authority and competence in prescribing medications.
• Preferred: Prior experience as a Primary Care Nurse Practitioner.
• Preferred: Proficiency with EMR documentation systems.
• Preferred: Strong background in preventive care and health promotion.
• Preferred: Effective patient education and communication skills.

Responsabilidades

• Conduct comprehensive patient history, physical examinations, and health assessments.
• Diagnose and treat acute and chronic primary care conditions.
• Develop, implement, and evaluate individualized patient care plans.
• Manage chronic diseases such as hypertension, diabetes, and COPD.
• Prescribe medications and monitor therapeutic outcomes and side effects.
• Order, interpret, and follow up on diagnostic tests and laboratory results.
• Provide preventive care, health screenings, and immunization services.
• Educate patients and families on disease management, lifestyle modification, and self-care.
• Document all patient encounters in the EMR system.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate care.
• Maintain compliance with clinical protocols, regulatory standards, and practice guidelines.
