Job summary

Monadnock Care Solution Physical Therapy in Highland Park, NJ is seeking a dedicated Physical Therapist for a full-time, onsite role. You will evaluate patients, design and implement personalized treatment plans, and collaborate with a multidisciplinary team to improve mobility and function.

You will work with a diverse patient population across acute and chronic conditions, using evidence-based methods and education to empower patients and caregivers.

Qualifications

• Bachelor's degree in Physical Therapy and NJ license.
• BLS certification from the American Heart Association.
• Strong communication and interpersonal skills.
• Proficient computer skills for records management.
• Ability to build and maintain patient relationships.

Responsibilities

• Conduct thorough evaluations and design personalized treatment plans.
• Implement and adjust programs for orthopedic, pediatric, neurologic and sports cases.
• Deliver excellent patient service and engage patients in recovery.
• Collaborate with a multidisciplinary team to optimize outcomes.
• Provide home-based therapies aligned with physician directives.
• Educate patients and caregivers on protocols, exercises, and lifestyle changes.
• Meet or exceed utilization and performance metrics.
