Headstrong Rehabilitation & Wellness is seeking a skilled Physical Therapist to join our team in Perth Amboy, New Jersey. This full-time role delivers rehabilitation services directly in patients' homes, providing personalized care with a focus on quality and compassion.

The ideal candidate holds a New Jersey PT license, a Bachelor's degree in Physical Therapy, and at least 2 years of clinical experience in physical therapy, preferably outpatient orthopedics. CPR/BLS certification is required.

Qualifications

• A Bachelor's degree in Physical Therapy or a related field from an accred ited educational institution.
• Current state licensure as a Physical Therapist (PT) in New Jersey.
• Two (2) years of clinical experience in a physical therapy setting, preferably outpatient orthopedics.
• Certification in CPR and basic life support.

Responsibilities

• Examine and evaluate patients with functional limitations to develop comprehensive treatment plans.
• Implement treatment interventions based on clinical findings and best practices.
• Document all patient interactions, evaluations, treatment plans, progress notes, and discharge summaries.
• Collaborate with a multidisciplinary team to ensure comprehensive care and timely communication.
• Conduct thorough assessments including history reviews to determine effective treatment strategies.
• Engage in ongoing professional development and compliance with policies and state regulations.
• Prepare and participate in discharge planning and follow-up care for patients.
• Attend and contribute to team meetings to improve patient access and care quality.

Key Responsibilities

• Implement treatment interventions based on clinical findings, patient presentations, and evolving best practices in physical therapy.
• Document all patient interactions, evaluations, treatment plans, progress notes, and discharge summaries accurately and timely to maintain compliance with regulations and standards.
• Collaborate with a multidisciplinary team of healthcare professionals to ensure comprehensive care and timely communication regarding patients' changing conditions.
• Conduct thorough assessments including history reviews and specific physical assessments to determine the most effective treatment strategies.
• Engage actively in ongoing professional development, training, and compliance with corporate policies and state regulations.
• Prepare and participate in discharge planning and follow-up care, ensuring a smooth transition for patients as they progress.
• Attend and contribute to facility meetings and team initiatives aimed at enhancing patient access and care quality.

Patient Population / Client Focus

As a Physical Therapist at Headstrong Rehabilitation & Wellness, you will primarily work with a diverse population of patients recovering from surgeries, injuries, strokes, and chronic conditions. Our patients range in age and ability, requiring individualized plans that address their specific rehabilitation goals. Your ability to build rapport, communicate effectively, and adapt treatment approaches will be crucial in helping patients achieve their maximum potential in the comfort of their homes.
