Job summary

Capital Area Health Network (CAHN) seeks a Patient Navigator to support colorectal cancer screening within a Virginia FQHC. You will coordinate patient care, help overcome barriers, and document in the EHR while protecting confidentiality.

On-site in Richmond, VA, the role requires strong communication, cultural sensitivity, and collaboration with care teams. Four years of clinical experience and licenses are preferred.

Qualifications

• Bachelor’s degree or equivalent combination of education and experience.
• LPN or RN preferred.
• Minimum of four years of related experience; oncology training preferred.
• Clinical experience; case management preferred.

Responsibilities

• Communicate with patients, families, and caregivers.
• Coordinate navigation and care across clinics.
• Assist with scheduling, referrals, transportation, interpreters.
• Document interactions and care plans in the EHR.
• Maintain navigation tracking tools and monthly reports.
• Ensure HIPAA confidentiality and patient privacy.
• Develop community linkages and referrals.
