Job summary

QuadMed seeks an Optometrist for onsite care at West Allis and Lomira. The role is 0.7 FTE, involving comprehensive eye examinations, diagnosis, treatments, and prescription of lenses or meds as needed.

Responsibilities include documenting history, performing testing, and coordinating with optical staff. Ideal candidates hold a Doctor of Optometry with a current license, Tennessee/Treatment Therapeutic Agent certification (TPA), and an active CPR/BLS credential, with an NPI on file before start

Qualifications

Four-year Undergraduate Degree.
Doctorate of Optometry from Accredited School of Optometry.
1 year of experience preferred but not required.
Current unrestricted Optometric License.
Therapeutic Pharmaceutical Agents (T.P.A.).
Must have an NPI prior to employment start date.
Current CPR (BLS) certification or within 60 days of hire.

Responsibilities

Conducts and documents patient history including chief complaint and relevant ocular health.
Performs required testing and documents results.
Interprets data from clinical exam and makes clinical diagnoses.
Prescribes corrective lenses and therapies as appropriate.
Communicates with optical staff to ensure timely service.
Maintains patient confidentiality and HIPAA compliance.
