Job summary

QuadMed in Wisconsin seeks an Optometrist to diagnose, manage, and treat eye conditions, prescribe lenses, and provide therapeutic care. You will perform testing, document findings, and collaborate with optical staff to deliver unhurried, patient-centered care.

The onsite position is 0.7 FTE at the West Allis and Lomira locations. A Doctor of Optometry from an accredited school, current unrestricted license, TPA, and NPI are required, with CPR/BLS to be obtained within 60 days of hire.

Qualifications

• Proven ability to work effectively with diverse populations and a commitment to inclusion.
• Must hold a Doctor of Optometry from an accredited school.
• Current unrestricted Optometric License and Therapeutic Pharmaceutical Agents (TPA) certification.
• Must have an NPI prior to start and CPR (BLS) certification (or within 60 days).

Responsibilities

• Diagnose, manage and treat ocular conditions and diseases; prescribe lenses and therapies.
• Perform and document history, testing, and clinical findings; interpret exam data.
• Operate ophthalmic equipment and communicate results to staff and patients.
• Maintain patient confidentiality and comply with HIPAA; participate in team meetings.
