Job summary

CompuGroup Medical seeks an Onsite Medical Assistant – Care Coordinator to support Medicare RPM and CCM patients. This on-site role focuses on high-volume outreach, documentation in EHR, and verifying patient data while guiding patients through their care plans.

The position emphasizes patient engagement, education on RPM usage, and coordination of device enrollment. Monday–Friday, no evenings or weekends, with strong growth opportunities in digital health.

Qualifications

• Active National Certified Medical Assistant required (NCMA/NRMA).
• Minimum 2 years of healthcare-related experience.
• Minimum 1 year in Chronic Care Management services.

Responsibilities

• Make 60+ outbound calls per day to eligible patients.
• Accurately document call outcomes using EHR and other systems.
• Conduct monthly check-in calls for RPM and CCM patients.
• Review and monitor vital signs from RPM devices.
• Explain RPM program benefits to patients.
• Educate patients on RPM device usage and enrollments.
• Conduct structured health assessments and follow care plans.
