Job summary

The position requires a Master’s degree in social work, counseling, psychology or related field and state licensure (LCSW, LPC, MFT, or ACD) with a commitment to patient education and interprofessional collaboration.

Qualifications

• Master's degree in social work, counseling, psychology, or a related field.
• State licensure as LCSW, LPC, MFT, or ACD, or other licensure may be acceptable as approved by leadership.

Responsibilities

• Implements counseling/therapy for assigned patients in individual and group settings.
• Interviews and assesses patients and families, caregivers, and/or legal representatives.
• Assists with discharge planning and identifies community-based resources; coordinates referrals as part of the discharge plan.
