Job Responsibilities

• Perform assessments on hospice patients.
• Must have active NY State Nursing License and at least 2 years RN experience.
• Experience in Hospice, Palliative Care, homecare, Oncology, Infusion, Med Surg, or Telemetry is a plus.
• Manage a caseload of 15-20 patients weekly.
• Provide wound care, PICC line management, infusions, Foley care, and midline care.
• Collaborate with interdisciplinary teams.
• Knowledge of CMS Hospice regulations.
• Conduct utilization reviews and educate families on hospice benefits.
• Maintain patient comfort and quality of life.
• Assess pain and assist with pain management.
• Document all evaluations clearly and accurately.
• Update Plans of Care as needed.
• Coordinate community services for patients.
