Job summary

Humana in Milwaukee (Aurora St. Luke's Medical Center) is seeking an Onsite Care Coordination Nurse to support newly admitted members with face-to-face assessments and discharge planning in a hybrid role.

You will collaborate with providers to ensure safe transitions from hospital to home and identify gaps in care. The role requires Wisconsin RN license and at least 3 years of acute care nursing, with strong skills in discharge planning, documentation, and patient education.

Qualifications

• Active Wisconsin RN license required.
• Minimum of 3 years recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Experience with Microsoft Office applications and EMR systems.
• Must have valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive face-to-face assessments for newly admitted members at acute care facilities.
• Support discharge planning from admission through post-discharge and coordinate follow-ups.
• Identify gaps in care and develop strategies to address issues.
• Collaborate with providers and care teams for timely, coordinated care and discharge planning.
• Provide education on diagnoses, medications, and preventive measures; ensure timely documentation.
