Evolve Treatment Centers is seeking a Mental Health Therapist for our 6-bed adolescent residential center in Encinitas, CA. You will provide individual and family therapy, conduct biopsychosocial and risk assessments, and develop comprehensive treatment plans for teens aged 12 to 17.

Role includes group facilitation, on-call coverage, and collaboration with a multidisciplinary team to coordinate care and discharge planning.

Qualifications

• Associates with AMFT/ACSW/APCC or licensed LMFT/LCSW/LPCC in California
• Must possess a valid California Driver's license
• Must pass LiveScan background check and clear federal and state criminal background checks
• Must be willing to interact with COVID-positive clients

Responsabilités

• Act as the point-person for each assigned client and their family during treatment
• Provide individual and family therapy to each client at a weekly frequency commensurate with the current level of care
• Facilitate 1-2 groups per week and complete group notes in the medical record
• Orient new clients and their caregivers to treatment and the program structure
• Facilitate Biopsychosocial Assessments and Risk Assessments for clients upon admission
• Create comprehensive treatment plans for clients, monitor progress and adjust, as needed
• Complete all clinical documentation within required timelines and in line with Evolve standards
• Participate in the clinical on-call rotation and provide phone-based support to on-site staff
• Communicate medical necessity for clients to the Utilization Review team to ensure appropriate insurance coverage
• Support discharge planning, including after-care and living arrangements
• Coordinate with Program Director on discharge dates
• Assess client belongings for safety and clinical relevance
• Identify interventions and communicate to the Treatment Team
• Update safety plans as needed per policy
• Provide updates during Treatment Team and weekly psychiatry meetings
• Attend trainings and fulfill mandated reporting
• Ensure Outcomes Program assessments are completed on time
• Interpret outcomes data for differential diagnosis and planning
• Other duties as assigned by the Program Director
