Job summary

Nichols College is seeking a dedicated case manager to provide services to families, assess needs, and co-create case plans. You will work with partners, complete essential documentation, and attend court hearings as needed. Qualifications

  • Bachelor's degree in social work, psychology, sociology, education or related field.
  • Clean background check and valid driver's license with good driving record.
  • Ability to build trust with diverse populations and maintain boundaries.
  • Excellent writing and documentation skills.
  • Strong judgment, independence and deadline orientation.
  • Ability to work independently and follow instructions.
  • Strong organizational skills and attention to detail.
  • Compassionate, empathetic and able to handle high-stress situations.

Responsibilities

  • Provide case management for a caseload of families; develop written case plans with family input.

  • Transfer cases between units with required documentation and home visits.
  • Present professionally to staff, clients and community; support teamwork.
  • Travel throughout state and occasionally out of state using agency vehicles.
  • Maintain confidentiality and regular attendance.

  • Provide case management services to an assigned caseload of families for up two years. Assess each child and family’s strengths, needs, goals, and objectives, involving the family in creating a written case plan. Maintain regular contact through home visits and provide supportive services.
  • Collaborate with service providers during case planning and throughout case progression.
  • Complete all required paperwork and documentation promptly and in accordance with agency policies and procedures.
  • Prepare for and attend court hearings; provide testimony as needed.
  • Complete timely transfers of cases between units, including required paperwork and joint home visits with receiving case workers.
  • Present yourself professionally to agency staff, clients, and the community; support teamwork and uphold the agency’s mission, values, and goals.
  • Travel throughout Ohio—and occasionally out of state—using agency-provided vehicles.
  • Maintain confidentiality of case and agency information, following state and agency policies and procedures.
  • Demonstrate regular and predictable attendance.
Qualifications and Skills:
  • Bachelor’s degree in social work, psychology, sociology, education or a related human services field.
  • Clean criminal background check and valid driver’s license with a good driving record.
  • Ability to establish trust, rapport, and ethical boundaries with diverse populations.
  • Excellent writing and documentation skills.
  • Sound judgment and decision-making abilities.
  • Ability to work independently and follow written/verbal instructions.
  • Strong organizational skills with the ability to manage time effectively and meet deadlines.
