- Licensed RN or LPN with pediatric experience.
- Ability to provide care for medically complex children at home.
- Comfort with home-based assessments and family education.
Responsibilities
- Provide skilled nursing care in home and school settings.
- Assess and monitor patient condition and communicate changes to the care team.
- Administer medications and prescribed therapies.
- Provide trach, ventilator, airway and G‑tube care as needed.
- Maintain accurate documentation and educate families.
- Follow individualized care plans and ensure a safe care environment.
