Innovate Rehab and Wellness in Moberly, MO is seeking a Licensed Occupational Therapist to join our Skilled Nursing Facility team. This full-time role offers a Monday–Friday schedule with a focus on one-on-one therapy and patient-centered care, delivering individualized treatment plans and helping patients regain independence.

You will collaborate with a multi-disciplinary team including PTs and SLPs, document progress, stay current with best practices, and participate in outcomes-driven care

Qualifications

• Master’s or Doctorate in Occupational Therapy (accredited program).
• Valid state OT license.
• Experience as an OT preferred (new grads welcome).
• Strong communication and problem-solving skills.
• Passion for patient-centered care.
• CPR certification preferred.

Responsibilities

• Conduct OT evaluations and document progress.
• Develop and adjust personalized treatment plans.
• Help patients improve motor skills, coordination, and daily function.
• Educate patients and caregivers on exercises and devices.
• Collaborate with PT, SLP, physicians, and the care team.
• Stay current with OT best practices and research.
