Job summary

Innovate Rehab and Wellness in Moberly, MO is seeking a Licensed Occupational Therapist to provide high-quality OT services in a skilled nursing facility. The role focuses on individualized treatment plans, patient independence, and improving daily function while collaborating with a full care team.

The OT will evaluate patients, develop plans, and train patients and caregivers on exercises and assistive devices, ensuring safety and optimal outcomes with a patient-centered approach.

Qualifications

• Strong communication and problem-solving skills are required.
• Experience preferred; new grads welcome.
• CPR certification preferred (if applicable).

Responsibilities

• Conduct thorough OT evaluations.
• Develop and adjust personalized treatment plans.
• Use therapeutic techniques and adaptive equipment.
• Train patients and caregivers on exercises and assistive devices.
• Educate on strategies to improve function and safety.
• Collaborate with PT, SLP, physicians, and the full care team.
• Document patient progress and treatment changes accurately.
• Stay current with OT best practices and research.
