Job summary

Sevita's Mentor Community Services in Phoenix is hiring Direct Support Professionals to provide one-on-one care for adults in home-based settings under IHSS/HCBS programs. You’ll support daily living activities, health maintenance, and community participation in family homes.

This non-exempt, hourly role offers starting pay of $15.15 per hour, with flexible full-time or part-time shifts. Ideal candidates have a High School diploma, at least 18 years old, and basic computer skills to document

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Accurately document progress and activity; reviews records and logs to stay abreast of changes in service plans; maintains confidentiality.
• Makes cost-effective and appropriate purchases within the budget of the individual served and according to their choice/preference.

Skills
