Job summary

CenterWell Home Health is seeking a Home Health Registered Nurse to deliver one-on-one skilled nursing care in patients’ homes, manage care plans with physicians, and monitor recovery to help patients regain independence.

Responsibilities include administering medications, wound care, patient assessments, education for patients and families, supervising Home Health Aides, and coordinating with the Clinical Manager.

Qualifications

• Current and unrestricted RN licensure.
• Med surg, ICU, ER, or acute care experience.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing care to patients in their homes, including medications, wound care, and assessments.
• Develop and implement individualized care plans with physicians.
• Monitor patient conditions and report changes.
• Educate patients and families on disease management and treatments.
• Maintain accurate records and coordinate with healthcare professionals.
• Report progress to physicians and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.
