Job summary

CenterWell Home Health seeks a Registered Nurse to provide skilled, one-on-one nursing care in patients' homes under the Clinical Manager. You will administer medications, perform wound care, and coordinate with physicians and clinicians to support recovery.

You will develop individualized care plans, monitor conditions, educate families, and oversee Home Health Aides to ensure proper documentation and patient safety, enabling patients to regain independence at home.

Qualifications

• RN licensure current and unrestricted.
• CPR certification required.
• Experience in Med-surg, ICU, ER or acute care.

Responsibilities

• Provide skilled nursing care to patients in their homes, administering medications, wound care, and patient assessments.
• Develop and implement individualized care plans with physicians and other professionals.
• Monitor patients' conditions and report changes to the physician and Clinical Manager.
• Educate patients and families on disease management, medications, and treatment options.
• Maintain accurate records of patient care and coordinate with other professionals.
• Report patient care and condition progress to physician and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.
