CenterWell Home Health is seeking a Home Health Registered Nurse to deliver one-on-one skilled nursing care in patients' homes, under the Clinical Manager. You will collaborate with physicians and a multidisciplinary team to implement care plans that support recovery and independence.

The role requires RN licensure, CPR, a valid driver’s license, and prior Med-surg/ICU/ER experience. A $10,000 sign-on bonus is offered; driving responsibilities are included and travel is expected.

Qualifications

• Med surg, ICU, ER, or acute experience.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license, auto insurance and reliable transportation.

Responsabilités

• Provide skilled nursing care to patients, working onsite in their homes, including administering medications, performing wound care, and conducting patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes.
• Educate patients and their families on disease management, medication, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and condition progress to patient's physician and Clinical Manager.
• Oversee the work of Home Health Aides and monitor their documentation.
