Job summary

Home Health Care, Inc. seeks a Certified Occupational Therapy Assistant (COTA) to deliver one-on-one therapy in patients’ homes under the supervising OT’s plan of care. You will help patients rebuild skills for daily living and independence.

You’ll work with caregivers and clinicians to implement treatment plans, monitor progress, and adjust interventions. Competitive pay with mileage reimbursement and flexible FT/PT options offered.

Qualifications

• Must hold Minnesota Certified Occupational Therapy Assistant (COTA) license.
• Strong understanding of ADLs and functional rehabilitation.
• Excellent patient and caregiver communication skills.
• Ability to independently organize your workday.
• Strong collaboration with supervising Occupational Therapists.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide one-on-one Occupational Therapy treatments in patients' homes.
• Carry out treatment plans established by the supervising OT.
• Address ADLs, functional mobility, coordination, and upper-extremity function.
• Provide therapeutic activities and exercises.
• Reinforce home exercise programs.
• Teach adaptive and compensatory strategies.
• Educate patients and caregivers.
• Monitor progress and communicate changes to the OT.
• Collaborate with nurses, PTs, SLPs, physicians, and interdisciplinary clinicians.
• Complete timely clinical documentation.
