Job summary

Home Health Care, Inc. is seeking a Certified Occupational Therapy Assistant (COTA) to deliver one-on-one OT treatments in patients\' homes throughout the Twin Cities metro area. You\'ll travel between patients, implement OT plans, and educate patients and caregivers.

You\'ll work with an OT supervisor and other clinicians, enjoying independence with strong clinical support. Full-time or part-time options are available, with paid per visit compensation and mileage reimbursement.

Qualifications

• Active Minnesota COTA license.
• Strong understanding of ADLs and functional rehabilitation.
• Excellent communication and relationship-building skills.
• Ability to work independently.
• Strong organization and time management.
• Confidence providing care in patients\' homes.
• Strong collaboration with supervising OTs.
• Ability to recognize and communicate changes in patient status.
• Valid driver\'s license and reliable transportation.

Responsibilities

• Provide one-on-one Occupational Therapy treatments in patients\' homes.
• Carry out individualized treatment plans established by the OT.
• Help patients improve activities of daily living.
• Provide therapeutic activities and exercises.
• Address upper-extremity strength, coordination, and functional use.
• Reinforce home exercise programs.
• Teach adaptive and compensatory techniques.
• Educate patients and caregivers.
• Reinforce safe use of adaptive equipment.
• Monitor patient response to treatment and communicate changes in status.
• Report progress to the supervising OT.
• Collaborate with PT, nursing, SLP, physicians, and interdisciplinary clinicians.
• Complete timely and accurate clinical documentation.
