American Oncology Partners, PA is seeking a registered nurse for patient‑centered telephone triage and care coordination. You will assess patient inquiries, route to the appropriate clinician, and document decisions. This role supports hospice and home health questions and requires collaboration with physicians and nursing staff.

The position may involve multi‑site coverage, occasional overtime, and travel between clinic locations as needed. A valid RN license and CPR/BLS are required.

Qualifikationen

• RN license in state of employment.
• Nursing degree required.
• Experience with hematology/oncology patients preferred.

• Answer patient calls and home health agency inquiries using nursing assessment to determine urgency and route to appropriate staff.
• Pull patient charts and document decisions; discuss abnormal labs or tests with physicians as needed.
• Coordinate care with physicians and nurses to ensure proper patient care and patient satisfaction.
